Terms and Conditions
1. Appointment Booking & Accuracy
Clients are responsible for selecting the correct service at the time of booking. If an incorrect service is booked, additional fees may apply or the appointment may need to be rescheduled. Appointment duration and fees are based on the service selected at the time of booking.
2. Identification Requirement
All clients must present a valid, government-issued photo ID at the time of service when required. Failure to provide proper identification may result in appointment cancellation without refund.
3. Consent & Authorization
By booking an appointment, you authorize Vessel Health & Wellness to perform specimen collection, fingerprinting, and related services associated with the selected appointment in accordance with applicable laws, regulations, and industry standards. Additional written consent may be required at the time of service.
4. No Medical Advice
Services provided by Vessel Health & Wellness are non-diagnostic and do not constitute medical advice, diagnosis, or treatment. Clients should consult a licensed healthcare provider for medical concerns.
5. Drug Testing & DNA Testing Disclosures
Where applicable, results are processed by certified third-party laboratories. Vessel Health & Wellness does not guarantee specific outcomes or result timelines. Legal DNA testing follows strict chain-of-custody procedures and cannot be altered once initiated. Results are released in accordance with laboratory policies and, where applicable, Medical Review Officer (MRO) procedures.
6. Prenatal & Gender Reveal Testing
Prenatal and gender reveal DNA testing is offered for informational purposes only unless otherwise specified. Vessel Health & Wellness does not provide medical interpretation of results. Eligibility for prenatal testing is subject to laboratory criteria.
7. Fingerprinting Services
Fingerprint quality and acceptance are determined by the requesting agency. Vessel Health & Wellness is not responsible for rejections due to agency-specific standards or technical requirements.
8. Mobile & On-Site Services
For mobile or on-site appointments, clients must ensure a safe, clean, and appropriate environment for service delivery. Vessel Health & Wellness reserves the right to refuse service if conditions are unsafe or unsuitable. Service start times may be adjusted based on site readiness, access, or safety considerations.
9. Cancellations & No-Shows
Cancellations or rescheduling must be made at least 24 hours in advance. No-shows or late cancellations may be charged up to the full appointment fee.
10. SMS Terms & Conditions
By opting in to receive SMS messages from Vessel Health & Wellness, you agree to receive the following types of text messages:
Appointment reminders (upcoming visit notifications)
Appointment confirmations (booking or rescheduling updates)
Service-related notifications (instructions, preparation details, or follow-ups related to your scheduled service)
Customer care communications (questions, clarifications, or coordination regarding your appointment)
Message frequency:
You may receive up to 4 text messages per month, depending on your level of interaction and the number of appointments scheduled.
Carrier fees:
Message and data rates may apply.
Opt-out:
You may reply STOP at any time to opt out of SMS messages, or reply HELP for assistance.
Data use:
Vessel Health & Wellness does not share, sell, or transfer mobile opt-in information to third parties for marketing or promotional purposes.
11. Privacy
Client information is handled in accordance with our Privacy Policy and applicable privacy laws. Sensitive information is collected only when necessary and through secure methods.
12. Limitation of Liability
Vessel Health & Wellness is not responsible for decisions made by third parties based on test results, including employers, schools, courts, or government agencies.
